Followed is a temporary physical ailment that is triggered after leaving a village with negative Standing and low Sanity. It lasts for 30-50 days, and causes a group of natives to confront the party every time the player loots a shrine or stone statue.

First the group of natives will demand tribute, triggering a trading page that allows the player to gift items worth enough value for them to leave peacefully. The player can also avoid combat by rolling a successful Tactics. If the player outright refuses the natives' demands, they will attack as enemies in a group of 3 to 6. Killing the natives rewards the players with ~5 items of loot typically found in villages.
